Project details: 

The Melton Community Based Ambulatory Care Centre (CBACC) is a new integrated centre to be constructed adjacent to the existing Melton Health facility.  APP has been engaged by the Department of Health and Human Services for all phases of the project from masterplanning through to completion.  The facility will bring together Djerriwarrh Health Services and six other project partners under the one roof in an integrated approach.  

Services to be provided include Womens, Childrens and Family Services, Aboriginal Services, Chronic Disease Management, Counselling and support services, plus Mental Health, Alcohol and drug-related Services,

The facility is designed to maximise integration between the project partners, through a series of shared spaces, to facilitate a more holistic approach to the treatment of clients.  The new 2,800m2 building will be connected to the existing adjacent Melton Health facility via an external canopy and walk way between the two buildings, together with a fully enclosed connection for staff access across both facilities.   

As part of the early design phase, APP reviewed and recommended the diversion of a large sewer main so as not to compromise the design and buildability of the new facility.

Melton CBACC will utilise an automated queuing system and integrated IT solution which connects all services providers.  As part of the project an IT meta-system will be developed to deliver a single point of access for client utilising various services, allowing integration of service delivery within a built environment and an IT environment to enable a holistic delivery of services.
